I was uncomfortable while standing playing my doubleneck, then Doug Brewer passed on to me advice Semie gave to him when he built his doubleneck. Wear the guitar as high as possible to get your elbow to bend about 90 degrees. I shortened the strap about eight inches and it worked fine.

This tactic has been working for me too. Of course the octave neck doesn't affect the center of gravity the way a 12 neck would.

As for the hypothetical Combo doubleneck - isn't a Combo semi-solid? I'd bet it would have a reasonably heavy body, for better or worse!

Heck yes...Combo 6/12!!! that would be a great 'build" thread. Somebody should build a whole series of D/Ns..... brassrail, Ventures 2, bluesbender, ect.
